  • APPLICATION FORM FOR YOUTH AND YOUNG ADULT PARTICIPANTS TORONTO CONFERENCE 93rd ANNUAL MEETING May 25th – 27th, 2017

    The 2017 Annual Meeting will take place in Midland, Ontario.

    Youth and Young Adults will gather on Thursday, May 25th at St. Paul’s United Church, 308 King Street, Midland, ON.

  • Meeting material is REQUIRED to be printed. Material will be mailed in May, ONLY to those who request it.
  • CONCURRENT PROGRAMS This year we will run separate programs for youth (age 12 – 17) and young adults (age 18 – 25). Participants in both programs will stay at the same hotel and share bus transportation to and from the meeting, but will gather separately for program.
  • PERSONAL INFORMATION PROTECTION AND ELECTRONIC DOCUMENTS ACT (PIPEDA) **Mailing addresses and phone numbers (unless unlisted) are under the public domain, while e-addresses are not. For ease of communication, the Conference and the Presbyteries wish to have your preferred email address. If you include your email address on this form, you are consenting to your e-address being distributed and used within The United Church of Canada. I consent to all the above information (excluding emergency contact information) being published in Toronto Conference/Presbytery Directories and available for use within The United Church of Canada. I have ensured that all the information is correct and as I wish it to appear.
  • Photo/Video Release: I/we grant permission to be photographed or videotaped while participating in all activities of the For Such A Time Retreat. I/We give permission for the use of photos or videos taken during the event/activities which may be published in print or posted online for download, in online photo albums, or on photo/video sharing sites such as Flickr or YouTube. Photos and videos may also be made available on CD, DVD, or Flash Memory devices at the discretion of Toronto Conference and The United Church of Canada.

  • Participants are responsible for administering their own prescribed medication as well as informing leadership when they take it for their own safety. Please include a description of possible side effects and the appropriate medical treatment.
  • ACCOMMODATION INFORMATION

    All youth and young adult participants will stay at Super 8, 1144 Hugel Ave, Midland ON L4R 0B1. 705-526-8288

    Participants will be paired with a room-mate. Room-mate assignments will be made by the organizers and will not be changed unless there are extraordinary circumstances. Security is provided during the night. Program leaders will also be staying on site and available if needed for emergencies, illness or disciplinary problems.

  • ARRIVAL AND DEPARTURE

    This year there will be a bus travelling from Toronto with pick-up locations in Alliston and Barrie to ensure that all youth and young adults have transportation to the meeting. The bus will leave Toronto at 9:30 a.m. on Thursday, May 25th to arrive in Midland in time for lunch and participation in a workshop on diversity. Bus registration and pickup times will be made available once you have been selected as a participant in youth and young adults at conference.

    If you do not come by bus you are required to arrive by 12 noon to St. Paul’s United Church.

    You will need to arrange your own transportation home on May 27th after the celebration of Ministries Service around 7:30 or 8 p.m. from St. Paul’s United Church. You may wish to arrange to travel home with ministry personnel or a lay representative from your community of faith. If not, it will be the responsibility of parents to arrange travel plans to get home. (Participants 18 and over may make their own travel arrangements).

    We will travel daily to and from the annual meeting by bus.

  • Toronto Conference Annual Meeting is being held at: ST. PAUL’S UNITED CHURCH, 308 King Street, Midland, ON.

    If the participant is under the age of 18, please let us know who is picking up the participant after the Celebration of Ministries Service.
  • IF EMERGENCY INFO AND/OR DROP OFF INFO CHANGES PRIOR TO THURSDAY, May 26th, PLEASE NOTIFY THE CONFERENCE OFFICE 416-241-2677, ext. 6241.

    CELL PHONE NUMBERS FOR LEADERSHIP WILL BE PROVIDED AT REGISTRATION SHOULD THE PICK UP INFORMATION CHANGE DUE TO AN EMERGENCY.

  • Standards of Event

    These standards are mandatory for participants under the age of 18 and encouraged for all participants. No alcohol. No drugs. No sexual activity. Those under the age of 18 who decline to follow these standards will be asked to leave the event. Access to pay per view and other user fee services (including in room snacks) in the hotel are prohibited and if used will be billed to the participant.

  • Parent’s Consent and Authorization for all Persons under 18 years of age (to be signed by Parent/Guardian)

    I/We, the undersigned parent(s) or guardian(s) of the minor named on the accompanying registration form, do hereby authorize adult workers with youth as agents of the undersigned, to consent to any emergency treatment or care which is rendered under the supervision of a physician or surgeon licensed in the Province of Ontario, whether rendered in the office of the physician or in a hospital. I/We, the undersigned parent(s) or guardian(s) of the minor named on the accompanying registration form do hereby consent to her/him/them engaging or participating in this event and all related activities. I/We agree that details of the event have been communicated to me/us including general activities, locations, and all other matters that I/we consider relevant.
  • This application should be submitted online and the Nomination Form also submitted online by the Chair (or designate) of your Community of Faith by Friday, March 17, 2017
